Stroke Care Co-ordinator (SCC)
We have defined an SCC as the following; however other titles may be in use:
- A registered healthcare professional with documented experience in stroke care
- Someone who is in regular contact with stroke patients and their carers in the community
- Their role may involve co-ordinating a range of longer-term care inputs on the patients' and carers' behalf (e.g. signposting, carrying out assessments etc)
- Works within a stroke service (as defined by the Royal Collage of Physicians guidelines)
Stroke System of Care Trial Eligibility
For the stroke system of care trial (LoTS-Care Trial) we have gathered SCC's who match the above definition. The trial includes SCC's who work alone and in community teams. For community teams to be eligible, patients need to be either allocated a key worker within in the team who will follow-up actions with a patient or the team needs to have a close working relationship to ensure patients are followed-up.
